A difficult surgery was performed successfully by a team of doctors headed by Dr. S.A. Wadood and Dr. P.K.D. Phukan, General Surgeon, down town hospital.

A 35 years old male patient was admitted in the city based down town hospital. He arrived in a state of shock at the emergency department. He had sustained a bullet injury with entry wound at the right upper abdomen just below the rib cage. There was no exit wound.

On arrival the patient was resuscitated in the emergency along with evaluation of the extent of injury. The bullet had entered just near the tip of the right 9th rib, through the right lower lung and the right lobe of the liver. It broke through the 9th rib at the back and had lodged in the muscles on the right side of the 9th and 10th spine.

Following resuscitation and evaluation the patient was taken up for emergency surgery. A chevron incision was used to access the abdomen. As the bullet was located near the 9th and 10th spine with the help of intra-operative X-ray and removed with a incision. It was a complex surgery due to the location of the bullet and the critical condition of the patient. The whole operation took around 5 hours.

Post Operatively patient was kept in the ICU for few days and subsequently in the ward. He was discharged on the 12th post operative day.
